Amy's Reflections

February 17, 2020

We had our Caravan Health ACO quarterly steering team meeting this past week. As many of you are aware we are participating in a Medicare Accountable Care Organization with a mission to reduce cost and improve quality for our Medicare patients. Each quarter when we meet we review many different statistics and quality metrics. One of the items that we looked at was the increase in annual wellness visits for our locations. Our ACO target is 50% or more of your attributed Medicare patients complete these visits annually. Medicare believes these visits are very important to the long-term health and wellness of this population as it give an opportunity to discuss, identify and prevent disease progression. I am so very proud of our teams and the work they do in impacting these patients’ lives. Special shout out to Kara Redenius and Tina Raimann for taking ownership of the process and increasing our numbers dramatically. Belmond moved from 21.9% in quarter 3 in 2018 to 64.1% in quarter 3 in 2019. This is incredible especially when our attributed lives also increased by 320 during that timeframe. Thank you for the difference you make!

This week is Random Acts of Kindness Week (2/16-2/22). In healthcare we perform thousands of acts of kindness on a daily basis. It is intertwined in the work that we do. It is in the comforting touch, the holding of a hand or the simple simile in the hallway. It is all those things that put our patients at ease and bring them comfort that you do without intentional thought. This week however, I would like to challenge you to be intentional in your random acts of kindness. Pick one person to impact each day with a random act of kindness. It could anything from sending a thank you out of the blue to calling a friend you haven’t seen in a long time to just touch base. The challenge is 7 random acts of kindness this week. I would love to hear how creative you can be with this challenge.
