Brooke Tapp - Daisy Award

Please consider Brooke Tapp for a Daisy Award nomination. I recently got to witness her connecting and developing a trusting relationship with a family while coordinating very complicated care for the husband. She navigated solutions and used critical thinking and sound judgement to create a plan for this patient and his wife. After receiving another call from Brooke with another solution for them on their care coordination, the wife was crying happy tears and the husband’s crummy day was turned around into a great day (he told us this). After seeing her work alongside this family for the last few weeks, I can see this type of compassion and commitment to the best care is the standard Brooke operates within. 

While this is just one patient, I know Brooke does this a lot, behind the scenes and rarely gets recognized or appreciated for her ability to coordinate cares and connect with the patients. She exemplifies the DAISY mission – she’s compassionate and is making profound differences in the lives of the patients she gets to serve.
