At the time of registration, you will be asked to provide a current insurance card or information about your current insurance carrier, as well as any specific forms or original documentation (claim forms, authorizations, referrals, etc.) required by your carrier.
Iowa Specialty Hospital will file insurance claims on behalf of our patients, but you are ultimately responsible for paying for the services provided.
At times your insurance company may delay processing your claims for various reasons. While you should receive information from your insurance company regarding the delay, we will also inform you of the situation.
As a service to our patients, Iowa Specialty Hospital contracts with a wide variety of insurance carriers to ensure their patients can be seen here and still be within their insurance network. Even though we do contract with plans, it is a good idea to contact your insurance carrier to verify participation prior to scheduling services. We contract with the following insurance companies:
